What is a BSSO?

Orthognathic surgery is a specialized field within oral, maxillofacial surgery (OMF surgery) that focuses on correcting position abnormalities of the jaw and face. The most common procedure in orthognathic surgery is the Bilateral Sagittal Split Osteotomy (BSSO). What is a BSSO? Who is this procedure suitable for?

Why a BSSO?

A BSSO is a surgical approach that focuses on correcting the abnormal position of the lower jaw. This procedure is often performed to correct functional and/or aesthetic problems related to an abnormal jaw position, such as an overbite or underbite. Incorrect positioning of the jaws can lead to problems such as difficulty biting off and reduced chewing ability. With a BSSO, the lower jaw can be moved into a new position to achieve a stable bite (occlusion) and an improved jawline and facial profile. This allows both functional and aesthetic problems related to an abnormal jaw position to be corrected.

 

How does the BSSO-procedure go?

During a BSSO, which is performed under general anesthesia, the lower jaw is examined at the jaw angles, just behind the last molar. The front part of the lower jaw, or the tooth-bearing part to which the chin is also attached, is moved to a new, pre-planned position. The jaw segments are then fixed with titanium mini plates and screws so that the jaw is whole again. In this procedure, the lower jaw is approached from the inside of the mouth so that there is no risk of visible scars on the face.

 

Planning of a BSSO

In order to perform the BSSO in a predictable manner, a good cooperation between the orthodontist and OMF surgeon from the first consultation is required. Together they assess the patient's specific needs, taking into account both functional and aesthetic considerations. A personalized treatment plan is drawn up. The plan usually consists of optimizing the dental arches and tooth position through braces treatment with brackets or invisible aligners. Once the dental arches fit together better, the BSSO is performed. After the BSSO, the orthodontist completes the brace treatment so that all tooth misalignments are corrected and people can smile with confidence.

 

Recovery after a BSSO

The recovery process after a BSSO usually goes quite smoothly. Patients may experience swelling and discoloration in the first two to four weeks. A temporarily adjusted diet will also have to be taken into account. In addition, the sensation of the lower lip and chin region may be temporarily reduced. Prior to the treatment process, the OMF surgeon will discuss detailed information with you about the treatment goals, treatment duration and treatment risks. 


In short, a BSSO is an effective procedure for correcting jaw deformities and can have a significant positive impact on the patient's quality of life. Due to the expertise and careful planning of the experienced orthognathic team, every phase of the treatment, from the initial consultation to the postoperative recovery, is carried out with the utmost care and attention.
